\u00c9tica y salud p\u00fablica \u2014 Propuestas para su integraci\u00f3n institucional<\/a><\/strong><\/p>\n\n\n\n

La salud p\u00fablica se ocupa de la salud colectiva. Trabaja con la incidencia de las enfermedades, con la prevenci\u00f3n de estas y con la promoci\u00f3n de la salud. Por ello, no suele atender a pacientes concretos en una consulta, ya que de eso se ocupa la medicina cl\u00ednica. Y, por esa raz\u00f3n tambi\u00e9n, los problemas y los conflictos \u00e9ticos de la salud p\u00fablica no coinciden con los de la \u00e9tica cl\u00ednica. Las pol\u00edticas de la salud p\u00fablica, por ejemplo, tienen un objetivo beneficente y no maleficente diferente al de la medicina cl\u00ednica porque no buscan el bien de un individuo concreto, sino el de la comunidad, lo que supone conflictos potenciales con las libertades individuales que van m\u00e1s all\u00e1 de la autonom\u00eda del paciente y un riesgo potencialmente mayor de da\u00f1o o maleficencia. Tambi\u00e9n tienen una relaci\u00f3n especial con la igualdad porque la salud p\u00fablica debe garantizar la equidad en la salud, lo que la enfrenta con los conflictos \u00e9ticos y pol\u00edticos propios de la justicia social. Por todo ello, la salud p\u00fablica requiere de una integraci\u00f3n\u00a0ad hoc<\/em>\u00a0de la \u00e9tica en todas sus pol\u00edticas.<\/p>\n\n\n\n

Utilitarianism, equality, and public health in a world in turmoil
<\/a><\/strong>One of the ethical conflicts facing public health is that which pits utilitarian ethics against egalitarian ethics, that is, the principle of maximizing aggregate health against the principle of moral equality. Public health codes and guidelines usually incorporate both of these principles because the ethical objectives of public health pursue the greatest health for the population and respect for the equality of all people and greater equity in health. The conflict between the two is particularly evident in contexts of health care rationing. In such contexts, the utilitarian ethic is the winner, contrasting with equality and equity in public health. The thesis I defend here is that this conflict stems from a deficient interpretation of both ethics, which should be corrected as follows: on the one hand, utilitarianism should be interpreted on the basis of the principle of equality and not against it, and, on the other, egalitarianism should include utilitarian decisions when these are the best way to interpret the principle of equality.<\/p>\n\n\n\n

One health ethics for emerging public health threats. Minding the gap between moral mandates and health extension to non-humans<\/a><\/strong><\/p>\n\n\n\n

The One Health approach encourages the normative consideration of human, animal and environmental health as a balance where these three interfaces are presented as interdependent. Thus, its ethical contribution may complement the scope of Public Health to address emerging threats that come into play non-human nature.
My research finds, from a non-anthropocentric view, three ethical pitfalls to be consistent with the moral mandate of One Health and to address emerging Public Health threats: health disparities between human and non-human animals; health disparities between animal and non-animals; and health disparities between individuals and ecological wholes.

The fascination of values: making use of ethics in public health<\/a><\/strong><\/p>\n\n\n\n

Public health measures intended to address collective health problems can be harmful or inconvenient for certain population groups. This hinders the effective enforcement of these measures, even when they are legal norms. In such cases, appealing to culturally accepted moral values can be an effective persuasion strategy to increase public support. This is an example of the usefulness of ethics for public health.

Ethical assessments and mitigation strategies for non-racial biases in AI-systems used during the COVID-19 pandemic<\/strong><\/a>
The main aim of this article is to reflect on the impact of biases related to artificial intelligence (AI) systems developed to tackle issues arising from the COVID-19 pandemic, with special focus on those developed for triage and risk prediction. A secondary aim is to review assessment tools that have been developed to prevent biases in AI systems. In addition, we provide a conceptual clarification for some terms related to biases in this particular context.

Bias in algorithms of AI systems developed for COVID-19: A scoping review<\/a><\/strong>
To analyze which ethically relevant biases have been identified by academic literature in artificial intelligence (AI) algorithms developed either for patient risk prediction and triage, or for contact tracing to deal with the COVID-19 pandemic. Additionally, to specifically investigate whether the role of social determinants of health (SDOH) have been considered in these AI developments or not. The Deliberated Decision in Children\u2019s Mainstream Cinema: On Socialization in the Discourse of Freedom<\/a><\/strong>
The purpose of this article is to highlight a trend in the narrative use of decisions that reinforces a widely accepted discourse of freedom. Through a discursive analysis focusing on contemporary mainstream screenwriting, it explores the relationship between the protagonist\u2019s decision and the climax of the story. pecifically, the article highlights a channel through which this discourse influences the sector of the public that is most sensitive to socialization processes: children and youth. With the normalized practice of screenwriting and without necessarily being aware of the fact, screenwriters can reproduce this discourse of freedom and thereby take part in reinforcing its social legitimacy.

Fraternidad pol\u00edtica y enfermedades raras<\/a><\/strong>
Las enfermedades raras reciben, en general, menos financiaci\u00f3n para su investigaci\u00f3n y tratamientos m\u00e9dicos que el resto de enfermedades. Al margen de explicaciones cient\u00edficas y econ\u00f3micas conocidas, existen dos grandes posiciones \u00e9ticas respecto a este hecho, la que lo aprueba por razones de \u00e9tica utilitarista, y la que lo condena insistiendo en razones igualitaristas y de derechos individuales. En este art\u00edculo, analizo los problemas de ambos tipos de justificaci\u00f3n moral, y propongo una alternativa \u00e9tica basada en la idea de fraternidad pol\u00edtica que, aplicada a las enfermedades raras, evita las peores consecuencias tanto del utilitarismo como del lenguaje del derecho individual a la salud y el mero igualitarismo.

El objetivo del presente trabajo es mostrar las ventajas de considerar al enfoque de Ronald Giere separado de lo que se ha llamado la familia semanticista en filosof\u00eda de la ciencia. La eficiencia no basta. An\u00e1lisis \u00e9tico y recomendaciones para la distribuci\u00f3n de recursos escasos en situaci\u00f3n de pandemia<\/strong><\/a>
Objetivo: <\/strong>Elaborar una herramienta de apoyo que ayude a tomar decisiones en el marco de la pandemia de COVID-19.
M\u00e9todo: <\/strong>Se parte de una b\u00fasqueda de diferentes recomendaciones \u00e9ticas surgidas en Espa\u00f1a sobre priorizaci\u00f3n de recursos sanitarios escasos en la pandemia de COVID-19, as\u00ed como de una revisi\u00f3n narrativa de modelos te\u00f3ricos sobre redistribuci\u00f3n en pandemias para definir una fundamentaci\u00f3n \u00e9tica. Finalmente, se extraen recomendaciones para su posible aplicaci\u00f3n en distintos \u00e1mbitos asistenciales.
Resultados:<\/strong> Se identifican tres principios \u2014 igualdad estricta, equidad y eficiencia \u2014 que se concretan en criterios de distribuci\u00f3n espec\u00edficos.
Conclusiones:<\/strong> Se propone un modelo de distribuci\u00f3n de recursos sanitarios escasos en situaci\u00f3n de pandemia que parte de un procedimiento de toma de decisiones y adapta los criterios de distribuci\u00f3n a los escenarios de la atenci\u00f3n sanitaria: atenci\u00f3n primaria, residencias sociosanitarias y atenci\u00f3n hospitalaria.
